Who can I enroll in my Family Dental Membership Plan?
You may enroll yourself, your spouse and your family members living in the same household. Any changes or additions of participants must be submitted by filling out a new application and approved by South Mountain Family Dental. Additional annual fees apply.
How long am I enrolled?
Activation of the Family Dental Membership Plan begins on the date of an approved application and when payment is received. The plan expires one year later to the date, no exceptions. The plan does not automatically renew.
We will always provide notification of your expiration date, so that you may choose to continue with the plan or let your benefits expire.
Is the Family Dental Membership Plan the same as dental insurance?
The South Mountain Family Dental Membership Plan is not insurance and no services are “covered” as in traditional insurance manner. Dental services are offered at reduced fees and full payment is required the day services are rendered. The membership plan may not be combined with other dental insurance and/or dental plans.
How is a Family Dental Membership Plan different than a cash discount?
Many dental offices offer cash discounts to patients without dental insurance. We have found, however, that many patients want something that will reduce their fees by more significant amounts, especially when treatment is needed. We have also found that patients enjoy the security of knowing that any visits they may require will receive the price reduction. The Family Dental Membership Plan offers savings of 30%-62% off of usual and customary fees. Much more than a typical 5% cash discount, and allows the participants to provide those savings for their whole family.
Do I have to pay the full annual fee all at once?
Yes. You will be responsible for paying your annual fee, in full, at the time of enrollment.
